  2. SDH, Kamakhya Nagar.

 

In DHH, Dhenkanal the ICTC has 2 units are working i..e,

  1. General clients and known as ICTC-1

  2. Pregnant ladies and known as ICTC-2.

 But one can approach either of them and get the same service.

General Feature :

 

What is HIV/ AIDS?

Ø      HIV is the acronym for “Human Immune deficiency Virus.

Ø      AIDS stands for  “Aquired Immune deficiency Syndrome”.

Ø      Aquired means :neither innate nor inherited, but transmitted from one infected person to another.

Ø      Immune is the body’s system of defence.

Ø      Deficiency means not functioning to the appropriate degree.

Ø      Syndrome means group of signs & symptoms.

Ø      AIDS is the advanced stage of HIV infection. HIV progressively destroys the immune system.

What Is Immune System?

Ø      Immune system defends the body.

Ø      WBCs are the most important parts of the immune system.

Ø      WBCs fight & destroy bacteria, fungi & viruses that enter the body.

How Does HIV Weaken The Immune System

Ø      When HIV enters the body WBCs are attacked by HIV.

Ø      HIV multiplies inside WBCs & infects the WBCs.

Ø      -Infected WBCs are eventually destroyed leading to a reduction in the no. of WBCs.

Ø      -It ultimately leads to greatly reduced immunity.

Ø      -An HIV infected person may take 6 months to 10 years (Avg. 8 yrs) to develop AIDS.

Ø      This is the crucial stage when they transmit virus from one person to another, as most of them unknown to their own HIV status.

Routes of Transmission

  1. Sexual intercourse

  2. Blood transmission

  3. Infected injection & syringes.

  4. Parents to child.

 How HIV is Not Transmitted

Ø      Through touch.

Ø      Mosquito bite.

Ø      Working together with an HIV+ person.

Ø      Living together with an HIV+ person & sharing common toilet, utensils, clothes, bed etc.

 

Signs & Symptoms of AIDS

Ø      Consistent fever without any apparent reason.

Ø      Loose motion, Diarrhea for longer than 1 month.

Ø      Unexplained weight loss.

Ø      Weakness.

Ø      Skin infection.

Ø      TB

Ø      Unexplained anemia.

Ø      Pneumonia.

Ø      Fungal infection.

 

Prevention, Cure & Treatment

Ø      HIV/ AIDS is not curable till date, hence it is calls as killer disease.

Ø      Only way of treatment is using general medicines & Dots for opportunistic infections like fever, loose motion, skin infection, TB & taking ART drugs. ART means “ Anti-Retroviral Therapy” which is now available in 3 medical colleges of Orissa. Patients have to go there for CD4 count test & if the doctor feels that, the patient is eligible now to take ART, then he can start.

Ø      The therapy is totally free of cost.

Ø      “Know AIDS for No AIDS” – Correct information is the only way to prevent AIDS. For correct information, counseling & testing ICTCs(Integrated Counseling & Testing Centers) are there in every District Head Quarters Hospitals & some Sub-Divisional Hospitals in Orissa.

Ø      There are 1361 no ICTCs across the state. In our District there are 2 units of counseling and testing centers-one is in Dist. Head Quarter Hospital, Dhenkanal and another is in SDH, Kamakshyanagar.

Ø      In DHH, Dhenkanal the ICTC has 2 units. One is meant for general clients and known as ICTC-1 another is for pregnant ladies and known as ICTC-2.But one can approach either of them and get the same service.

Ø      The ICTCs in Dhenkanal were opened in December,2005 and testing began from March,2006.In both the ICTCs there are 1councellor for counseling and 1 laboratory technician for HIV screening test. In both the centers pre-test counseling is followed by HIV test and with the consent of the client the test must be done. Then post-test counseling is done and the client receives his test report.ICTC-1is located at out patient department, room no-5 and its opening time is from 8am to 12noon and in after-noon from 3pm to5pm or as per the out-door timing.ICTC-2 is attached with O&G Department it is opening from 8am to 2pm.

Ø      Any person above 18yrs of age and below 75yrs of age can come to ICTC for counseling, testing and for any kind of information related to HIV/AIDS. If a person is found HIV +ve then necessary counseling is given to him and the person is referred to ART Centers for further treatment. The centre keeps in touch with the Hiv+ve   person till the end of his life.  

Women – the Silent victims of HIV / AIDS

Ø      Women generally get infected from spouse and know their status after the spouse is found +ve

Ø      ICTC-2 which is initially known as PPTCT (Prevention from parent to child transmission) deals with pregnant women.

Husban (HIV +ve)  +  Wife (HIV +ve ) = Child (HIV +ve)

Husban (HIV +ve)  +  Wife (HIV – ve ) = Child (HIV – ve)

Husban (HIV – ve)  +  Wife (HIV +ve ) = Child (HIV +ve)

Ø      One of the most reliable way to check the transmission is from mother to child

Ø      Every woman has the right to be a mother. We always can’t advice to terminate pregnancy in case of HIV +ve mother. And it is solely depends upon the mother. Whether she wants to terminate the pregnancy or not. But we can do proper counseling and can take some steps to check the transmission like

Ø      We can ensure for institutionalize delivery

Ø      Caesarean is better than vaginal delivery as in vaginal delivery there more chance of transmission. So we can convince the mother for Caesarean delivery.

Ø      Most important duty of ICTC-2 is to give Nevirapine to mother and child. Nevirapine  checks the transmission from mother to child.

A 200 mg pill of Nevirapine is given to the mother on the onset of labour. 6 ml Nevirapine (Oral suspension) is given to the newborn baby with in 2 hrs. of delivery
